Year 9 Diphtheria, Tetanus and Polio (DTP) and Meningitis ACWY vaccines

Dear Parent/Carer

The immunisation team are due to attend your child’s school in the next 3 weeks to vaccinate Year 9 pupils with the Diphtheria, Tetanus and Polio (DTP) and Meningitis ACWY immunisation.

If you have not already consented for your child to receive these vaccines please visit www.hdft.nhs.uk/year9form and use school code 137702 to give your consent. The last date to be able to give online consent is at 23.59pm on *22.03.21. If you have no internet access, please call the immunisation consent line on 01423 557693 and we can take your consent over the phone.

If you have already given your consent for your child to receive these immunisations then you do not need to do anything.

I am not sure if I have already given my consent, do I need to do it again?

If you are unsure if you have already given consent, please contact the immunisation team via email on [email protected] or call on 0300 003 2554 and a member of the immunisation team can check this for you.

My child is unable to attend on the day – how do I cancel/rebook?

If your child will not be in school on the planned session date, you do not need to contact us. If you have consented online, your child’s consent form will be automatically transferred to our catch up list and we will either catch your child up in school later this year or invite them to a catch up clinic.

I have a child in Year 10+ who is also outstanding their DTP and Men ACWY. Can they be caught up on the session date?

Where possible we will be trying to catch up older year groups on the session date, but this will vary depending on venues, staff availability and current covid restrictions. If you have previously consented for your child, they will be on our catch-up list and if there is the time they will be immunised while we are in school otherwise we will be offering a clinic venues.

Can we organise this vaccination at my GPs instead?

GPs are not commissioned to provide routine school-age immunisations and will refuse in the majority of cases.

Please note, we are a very small team, and it can take us up to 24 hours to respond to emails. Please do not contact your child’s school – they are not directly involved in the planning and delivery of the immunisation session.
